This is a very good point. In my head I had always imagined someone
taking the Full Defense only after they had been hit. Never just
straight away as an "immediate" action. You're right, there's no
downside. That is a problem.
I would say in this that if you've attacked, you lose your Full
Defense option until you either go again, or you get hit.
Alternately, just drop Full Defense altogether to avoid the headache,
and rely solely on tags and invocations to turn the fight around.
Even more alternately, disallow Full Defense as an "early" action, and
only allow it to be declared on your turn. This is probably the best
choice.
